The 55 year old Agra based shoe exporter, Ashwini Marwah had been gaining weight for the last few years. He was suffering from hypertension and diabetes and weighed about 130kgs. Therefore, he decided to undergo a bariatric surgery at a private hospital in Delhi to lose those extra kilos. However, he got the shock of his life when he was told that an adrenal tumor weighing 11.5kgs which is probably the largest in the world was completely occupying the right side of the abdomen.
As per the treating doctors, the largest tumors that have been reported in India so far weighed 6kgs which was removed in 2003.The largest tumor in the world weighed 7.5kgs which was removed from a Chinese patient in 2013. The private hospital has written to the Guinness Book of World Records to consider this entry. All the cases have been well documented and published in medical journals.
According to Dr. Anoop Mishra, Chairman at Fortis Hospital, “During diagnosis of Mr. Marwah, we found that he have a large abdominal tumor of size 30x25cm, but was unaware of the tumor expect for causing an increase in his weight, heaviness and dragging sensation on the right side of the abdomen since he remained asymptomatic. The tumor was located adjacent to the vital organs such as inferior venacava, kidney and intestines.”
A High Risk Candidate For Surgery
Marwah suffers from hypertension and type 2 diabetes and came for an evaluation for bariatric surgery at Fortis Hospital, Delhi. Dr. Anoop Mishra, the Chairman of Fortis Hospitals says that they found an abdominal tumor during evaluation and investigations. And the patient was unaware of the tumor, except for increase in the weight, heaviness and dragging sensation on the abdomen, he was asymptomatic. He says, “In this case, Marwah had a body mass index (BMI) of 48, hypertension, diabetes mellitus, and early changes of nephropathy. Hence, he was a high risk candidate for surgery. So he was evaluated extensively before the surgery. We had to make a large incision on his abdomen with regards to the giant size tumour. On removal, the tumour weighed around 11.5kgs, making it the largest adrenal myelolipoma ever to be excised. In this case, the biggest challenge was to salvage his right kidney since his kidney functions were already affected due to his hypertension and diabetes."
Bariatric surgeon Dr. Randeep Wadhawan added that the adrenal myelolipoma is a very rare benign tumour. Most lesions are small. They are asymptomatic and discovered incidentally. It is usually small, and non-functional in nature. It contains the mature adipose tissue as well as haematopoietic elements in variable amounts. The overall incidence is 0.05% to 0.2%. Ashwini Marwah said, "I had gained substantial weight in the past three months and was feeling very heavy from inside. Though there was no alarming symptom, I was taken by surprise when doctors told me that my weight gain is not due to Diabetes but a giant tumour.
