Overview: Fibroids

Fibroids, also termed as Leiomyomas or Myomas are almost always non-cancerous muscular growths i.e. tumors that grow in the wall of uterus.  Fibroids, in uterus, may grow as single or multiple tumors, often during childbearing years. The fibroids may vary in shape and size and appear as an apple seed or big like grapefruit.

They can develop at different location in uterus such as uterine wall or on its surface, attached to uterus by a stalk or stem-like structure. Sometimes these fibroid tumors become quite large and cause severe abdominal pain and heavy periods. Most women get Fibroids in childbearing age i.e. between 30 to 40 years but can develop at any age. Chances for the development of fibroids are more among women nearing 50 years of age. 

Fibroids are formed due to:

• Hormones such as Estrogen and Progesterone and cause thickening of the lining of uterus every month during menstruation
• Fibroids may develop and grow rapidly during pregnancy due to increased production of Estrogenand Progesterone 
• Genetic Changes - Many fibroids cause changes in genes that differ from those in normal uterine muscle cells.

Risk factors for development of fibroids

Other than childbearing or reproductive age, risk factors that can impact fibroid development include:

• Race – compared to other racial groups. Black women are more likely to have fibroids
• Family History or Heredity
• Early Menarche - Early pubertal development with onset of menstruation at an early age
• Delayed Pregnancy
• Genetic Alterations

The other risk factors for development of fibroids can be:

• Obesity
• Diet higher in red meat and lower in green vegetables, fruits and dairy
• Excessive consumption of beer or alcoholic beverages
• Vitamin D deficiency
• Parity – a protective effect for uterine fibroids

Symptoms

Symptoms of fibroid may include:

• Abdominal swelling and pain in lower abdomen
• Pelvic discomfort or pain

• Pressure or fullness in lower abdomen

• Pain during intercourse
• Increased Uterine cramping before and during menstrual periods
• Menstruation lasting longer than normal
• Heavy bleeding during periods
• Miscarriages or  Infertility
• Enlarged abdomen or uterus
• Constipation

Diagnosis

· Pelvic examination - may reveal an enlarged uterus or mass.

· Ultrasonography - uses sound waves to get a picture of your uterus to confirm the diagnosis and measure fibroids

·  Hysterosonography - uses sterile saline to expand the uterine cavity to get images of submucosal fibroids and uterus lining

·  Hysteroscopy -   performed to differentiate Intracavitary Myomas and Large Endometrial Polyps

·  Magnetic Resonance Imaging (MRI) – helps determining the number of fibroids, their size and vascularization.

·  Hysterosalpingography - uses a dye to highlight the uterine cavity and fallopian tubes on X-ray images.

Methods of treatment

Treatment of Fibroids include;

·   Watchful waiting – especially when there are no discomforting symptoms.  Since, fibroids grow slowly and shrink after menopause, doctors prefer to wait and watch

·   Medication
